This Funding Opportunity Announcement (FOA) invites submission of investigator-initiated Program Project (P01) applications. The proposed programs may address scientific areas relevant to the NIAID mission including the biology, pathogenesis, and host response to microbes, including HIV; the mechanisms of normal immune function system development and function; and immune dysfunction resulting in autoimmunity, immunodeficiency, allergy, asthma, and transplant rejection; and translational research to develop vaccines, therapeutics, and diagnostics to prevent and treat infectious, immune-mediated, and allergic diseases. Each P01 application submitted to this FOA must include at least two related research projects that share a common central theme, focus, and/or overall objective.

Description:
The burden of HIV in Zambia is based on data collected from the general population, with prevalence measured at 13.3% among individuals aged 15 – 49 years. Because little is known about the risk of HIV infection among key populations, this FOA aims to provide a mechanism that will tailor a surveillance system complimentary to population-based surveys for vulnerable groups. Special attention to sensitive sampling of key populations and measurement of risk behaviors illegal under Zambian law is needed in order to provide a better profile and understanding of HIV transmission in Zambia. Such data will also provide an evidence base for programs and policies targeting these populations and fill a void in the knowledge base of HIV epidemiology in Zambia. This information will be used by HIV programmers to target these populations more effectively with prevention, care and treatment, and, given that their sexual networks extend to the general population, mitigate HIV among all Zambians.

This FOA will support (strengthen) Faith Based Organizations in Zambia to provide support for HIV/AIDS service delivery in facilities that serve the religious communities (in facilities and communities they serve). Overall, this FOA will aim to support the Government of Zambia's response towards sustainable HIV epidemic control by: a) reducing new HIV infections, b) reducing morbidity and mortality among adults and children living with HIV; and c) strengthening health support systems. The awardee will be required to implement a core package of prevention, care, treatment, and laboratory services; this will result in reduced HIV transmission risk and improved quality of HIV clinical care. The awardee will ensure continuum of care through linkages between community and facility services with documentation of clinical outcomes. The awardee will be expected to support in-service training and mentorship through innovative, and cost efficient models and health systems strengthening at the facility, community, district, and provincial levels.

In the scale-up districts, the awardee will provide a package of services that will result achievement of the 90-90-90. The awardee will create site and community level demand for HIV prevention, targeted testing, counseling, and care services. For Sustained districts, the awardee will provide passive enrollment via HIV testing and counseling (HTC) on request or as indicated by clinical symptomatology, care and treatment
services, and essential laboratory services; TA for site-level services, including: targeted HIV testing services (HTS); prevention of HIV transmission among KP and PP; HIV care and treatment; TB/HIV prevention, diagnosis, and treatment; eMTCT; VMMC; and services to OVCs; and ensure gender integration across all technical areas.
