Today the Centers for Medicare & Medicaid Services (CMS) released an issue brief that highlights the key features of three approved health home models from Maryland, Rhode Island, and Vermont, tailored to individuals with opioid dependency. It identifies important considerations in developing opioid dependence-focused health homes, including: (1) leveraging opioid treatment program requirements; (2) promoting collaboration across multiple state agencies; (3) supporting providers in the process of becoming health homes; and (4) encouraging information sharing.

The issue brief is available under “Developing Health Home Population Criteria” at http://www.medicaid.gov/state-resource-center/medicaid-state-technical-assistance/health-homes-technical-assistance/guide-to-health-homes-design-and-implementation.html.
