CMS Releases Medicaid Health Home State Plan Option State Plan Amendment Template and Implementation Guide

View in browser | Distributed by Center for Medicaid and CHIP Services (CMCS) Today, the Centers for Medicare & Medicaid Services (CMS) release the state plan amendment (SPA) template and implementation guide for the Medicaid state plan option to cover health home services for Medicaid-eligible children with medically complex conditions, enacted as part of the Medicaid Services Investment and Accountability Act of 2019.  CMS has posted reference documents on Medicaid.gov or states to view, and any official SPA submissions must be submitted through the MACPro system.
